Medicare Payment Limit Lookup

Enter a HCPCS code or drug name to see what Medicare Part B allows per billing unit, the hospital outpatient (APC) limit for the same code, and how the payment limit has moved quarter by quarter over the past two years. Free for five drugs a day, no account needed.

CMS publishes a payment limit only for codes with a current Average Sales Price, so some codes return nothing. ASP, WAC, and AWP, and 10 years of history for all four, need a free trial.

Try an example:

The four prices a drug carries

  • ASP Average Sales Price

The volume-weighted average price manufacturers actually realize on a drug, net of most discounts and rebates. Manufacturers report it to CMS, and it is the basis for what Medicare Part B pays.

    Reported quarterly by manufacturers

  • WAC Wholesale Acquisition Cost

The manufacturer's published list price to wholesalers, before discounts and rebates. It is the common reference point for a new drug in the quarters before its first ASP.

    Refreshed weekly

  • AWP Average Wholesale Price

A published list-price benchmark, typically set above WAC, still used as a reference in many commercial and state program contracts.

    Refreshed weekly

  • Payment Limit Medicare Payment Limit

What Medicare Part B actually allows for a separately payable drug, generally ASP plus 6 percent. CMS publishes it quarterly.

Biggest Movers

Biggest Medicare Price Moves

The largest moves in the Medicare Payment Limit. Drops track biosimilar and generic entry; increases often mark sole-source or shortage drugs. Pick a window, and follow any drug through to its full record.

Figures are the change in the Medicare Payment Limit per billing unit; a subscription opens the full ASP, WAC, and AWP history behind each.

How It Works

From a Code to a Price Trend

Every drug's Medicare Payment Limit, and its last two years of history, are free to look up above, five drugs a day. A subscription removes the daily limit and adds current ASP, WAC, and AWP, and the full history behind each price.

  3. Unlock the full picture

How far back does pricing history go?

More than ten years, for every drug we cover. We keep each published quarter of ASP and the Medicare Payment Limit alongside the weekly WAC and AWP record, so you can follow a drug from where it sat a decade ago right up to the current quarter. Searching, the current Medicare Payment Limit, and its last two years of history are free to view for five drugs a day; ASP, WAC, AWP, and the full history come with a subscription.

Which prices can I trend over time?

ASP, WAC, AWP, and the Medicare Payment Limit can each be charted over time, on their own or side by side. They move on different schedules and for different reasons, so lining them up is the quickest way to see where list price and actual reimbursement have pulled apart, usually the clearest sign of biosimilar or generic entry. The payment limit’s recent trend is free to view; the rest come with a subscription.

How often is pricing updated?

WAC and AWP refresh every week, on Tuesdays. ASP and the Medicare Payment Limit follow the CMS quarterly schedule, taking effect in January, April, July, and October, and we apply any CMS corrections as soon as they are published. Whenever the files are released early, we post the new pricing ahead of its effective date, so you can see what is coming before the quarter turns. To see which limits actually moved when the latest file landed, and by how much, open the Medicare Part B reimbursement board.

What does a subscription include?

The full pricing picture on every drug we cover: current ASP, WAC, and AWP next to the Medicare Payment Limit, plus more than ten years of history for each, charted so you can compare drugs and biosimilars side by side. You can start with a 7-day free trial that opens all of it.

What can I see without a subscription?

Quite a bit. Searching is free, every drug code page shows the descriptor and publicly available coding detail, and the lookup above returns the current Medicare Payment Limit and hospital outpatient (APC) limit along with the last two years of the payment-limit trend, no login needed, for five drugs a day. ASP, WAC, AWP, and the full history behind each price are where a subscription comes in.

How many free lookups do I get each day?

Five drugs a day, no account needed. Looking up a drug you have already opened that day never counts again, and the count resets every day. Drug code pages are not limited — every code page stays free to browse. A subscription removes the daily limit and adds ASP, WAC, and AWP with the full history behind each price.
