The two codes answer different questions. The HCPCS code tells the payer which drug was administered and in what unit of measure. In most cases, yes. A complete claim includes the HCPCS code for the drug itself and a CPT administration code for the service of giving it, such as 96372 for a therapeutic injection or 96413 for the first hour of a chemotherapy infusion. New drugs often reach the market before CMS assigns them a permanent code. Until one is issued, the drug is billed under a not-otherwise-classified (NOC) code, such as J3490 for unclassified drugs, J3590 for unclassified biologics, or J9999 for chemotherapy drugs. The claim must also report the 11-digit NDC and the dose administered. CMS reviews new code applications quarterly, so a permanent code usually follows.
CMS updates the HCPCS code set quarterly, with new drug codes taking effect in January, April, July, and October.
